We provide comprehensive care when you’re an inpatient and after you leave the hospital. We partner with specialists in heart and vascular care, lung and respiratory care, senior health and orthopedics.

Scheduling Your First Appointment

Our outpatient rehabilitation office staff helps you schedule your first appointment. We offer flexible options. Most locations are open Monday through Friday, 7:00 am to 5:00 pm.

Please have your doctor's referral or prescription available when you call to make your appointment.

Your first appointment will take one hour. This lets your therapist fully evaluate your condition. Your therapist will outline the frequency and duration of your treatment during your first visit.

Follow-up appointments usually last 30 minutes.

Preparing for Your First Visit

You will need:

  • A prescription for outpatient therapy from your referring doctor.
  • Confirmation of your insurance coverage.

Contact your insurance company to understand:

  • Your benefits for therapy.
  • Necessary insurance referrals.
  • Required deductibles and co-pays.

If you need financial assistance, contact our billing department to learn about your options.

What To Expect

Please bring the following items with you to your first appointment:

  • Copayment
  • Health insurance card
  • Prescription from your physician

Dress appropriately. Wear loose clothing. If you have an upper body condition or injury, wear a tank top or T-shirt. If you have a lower body condition or injury, wear shorts or sweatpants.
